Outpatient Services
Creedmoor operates a broad range of outpatient services, both on campus and in the communities which we serve. The services offered include outpatient clinic treatment and residential services. Services are provided on a fee for service or pre-paid plan basis, depending on the needs and eligibility of consumers. Sites are staffed with psychiatrists, social workers, nurses, psychologists, rehabilitation staff and para-professional staff.

Specialty Services for Outpatients
- Assertive Community Treatment (ACT): This team provides mobile treatment and wraparound services for those patients in Eastern Queens who need support post-discharge to prevent re-hospitalization. The team provides active treatment and links to other needed community services.

- Bridger Program: This program provides linkage and support for inpatients with long hospital stays when the possibility of discharge becomes imminent. "Bridger" staff work with patients to enable them to move into the community with greater hope, confidence and an increased probability of success.

Intensive Case Management Program (ICM) provides an intensive level of supervision to those outpatients who are frequent system users and have specialized needs in the community. This program serves as a support and follows patients throughout outpatient and inpatient admissions. ICM staff provide coordination of services for consumers under “Assisted Outpatient Treatment” court mandate.
- Case Management Program: A "step-down" program for those patients who need support to function in the community, but do not require the level of intensity provided by the ICM program.

- Mobile Crisis Team (MCT): This team services individuals in Eastern Queens who are at risk or are experiencing a psychiatric or emotional crisis which requires mental health intervention, treatment and or support. They provide home visits, assessment of needs, psychiatric assessment and evaluation, supportive counseling, linkage to services, and short-term follow-up.
Areas Served; College Point, Bay Terrace, Whitestone, Flushing, Queensboro Hill, Bayside, Auburndale, Little Neck, Douglaston, Glen Oaks, Floral Park, Bellerose, Queens Village, Cambria Heights, Laurelton, Rosedale, Springfield Gardens and Jamaica. Hours of operation: 12 noon - 8:30 p.m., Monday thru Saturday.

Residential Programs: Residential programs range from the community residences (Oak House, Stepping Stone & Gateway Transitional Residence) to the home care of the Family Care program. Each program offers a unique set of advantages to consumers, promoting empowerment and self determination, while ensuring care and safety.

Stepping Stones Transitional Residence: This on-campus program has 48 beds and is a short term program for recently discharged inpatients. Residents attend clinic at the Queens Village Community Services site, or other specialized providers.

Gateway Transitional Residence: This on-campus 24 bed residence provides short term housing for recently discharged inpatients. Residents receive psychiatric services from Creedmoor's community programs or from other providers as needed. Referrals from CPC’s outpatient clinics and Queens Network’s CPEP programs are also accepted for temporary use beds.

Polaris House: A transitional 25-bed supervised community residence, with the goal of assisting each resident to develop the community living skills needed to move to more independent community placements.

Oak House Community Residence: A 24 bed state operated community residence for those consumers who need a more intensive housing program experience before leaving for sites featuring greater independence. It is located on campus.

Family Care: Family Care Homes provide 24 hour residential services in small family settings for consumers unable to function in more independent living arrangements. The program’s "personal care" component offers additional hands-on services for chronically disabled residents.
