Where should I send my CFR?
The CFR should be uploaded to OMH via the Internet. Go to the CFRS Home Page and follow the directions.
The signed certification schedules and financial statements should be mailed to the following address for OMH:

If you are funded by a local county contract, check with the county for any additional requirements.
If your agency also receives funding from other NYS agencies, refer to Section 2.0 of the CFR Manual.
When is the CFR due?
July 1, 2007 - June 30, 2008 CFR was due November 1, 2008.
January 1, 2008 - December 31, 2008 CFR is due May 1, 2009.
July 1, 2008 - June 30, 2009 CFR is due November 1, 2009.
January 1, 2009 - December 31, 2009 CFR is due May 1, 2010.
July 1, 2009 - June 30, 2010 CFR is due November 1, 2010.
A 30 day extension may be received by sending the Pre-approved 30-Day Extension Request form to all NYS agencies that require you to file a CFR. If you are funded by more than one NYS agency, the request must be sent to each funding agency. No response will be sent, therefore, you must retain a copy for your files. State Agencies will not be responsible for any loss of state aid which results from non-compliance with the required deadlines or late amendments.
Late submission of a CFR may result in a sanction or penalty being imposed against the service provider.

What types of equipment should be expensed on CFR-1, line 28 and CFR-3, line 13?
The equipment expensed on this line should include only those items with a cost of less than $1000 or a useful life of less than two years. Items with a cost of $1000 or more and a useful life of two years or more must be capitalized and depreciated; and the annual depreciation expense should be recognized each year on the CFR core schedules (CFR-1 through CFR-6).
This may be different than how you account for these items for reimbursement/claiming purposes (DMH-2 and DMH-3).
Purchases funded via rates, fees or net deficit funding must still be capitalized and depreciated on the CFR core schedules.
Where do I report fundraising?
Fundraising revenues and expenses should be reported on schedule CFR-2, in the ‘Other Programs’ column.
Operating expense for fundraising must be reported on CFR-3, as part of ‘Other Programs’ operating expenses. Agency administration is allocated to these expenses via the ratio value method.
Where do I report the cost for the audit of the CFR?
The cost for the audit of the CFR should be included on schedule CFR-3, on line 6 (Audit/Legal).
What is a non-allowable cost?
These are expenses that may not be considered reasonable, necessary and related to patient care. These costs may not be reimbursable. Please refer to Appendix X of the CFR Manual for additional guidance.
Costs that are not included in your budget are not necessarily considered non-allowable for CFR purposes. These costs may be considered non-funded.
How do I find the program/site identification number?
For OMH programs: Use the operating certificate for certified programs and the Facility-Unit code for non-certified programs.
Providers may use the Mental Health Provider Data Exchange (MHPD) web-based application to get permanent program/site identification numbers or make changes to current program/sites. For information about obtaining access to MHPD, please contact MHPD. It is likely that your agency already has access to MHPD.
You may also wish to check desk audits that you received from the OMH CFR Unit regarding previous CFR's. These might contain information regarding the correct program/site identification number, or may include a temporary number that you can use until a permanent one is assigned.
