2005 Patient Characteristics Survey (PCS)

The 2005 Patient Characteristics Survey form: front view and back view (both in PDF), and guidelines for completing the survey form (PDF) (HTML), are available in Portable Document Format (PDF). Software called Adobe Acrobat Reader is needed to read or print PDF documents. You may download a free Acrobat Reader from Adobe or call 1(800) 430-3586 to order a printed copy of the documents.

You can select a number of reports that reflect results of the 2005 PCS. Information is summarized at three levels - county (of provider), region (of provider), and state. Each level contains seven reports:

  1. Major Age Group by Program: Number of people served in each age group (Under 18 years, 18 to 64 years, 65 years and older) broken down by approximately 80 service programs.
  2. Race by Program: Number of people served in each ethnic group broken down by approximately 80 service programs.
  3. SMI/SED Status by Program: Number of people served who were SMI/SED, and the number who were not SMI/SED, broken down by approximately 80 service programs.
  4. Current Residence by Program: Number of people served in each residence type broken down by approximately 80 service programs.
  5. Detailed Age Group by Gender By Race: Number of people served in each age group (Less than 5 years, 5-12 years, 13-17 years, 18-20 years, 21-34 years, 35-44 years, 45-64 years, 65-74 years, 75 years or older) broken down by gender and ethnic group.
  6. Major Age Group by Various Criteria: Number of people served in each age group by: current disabilities, employment status, SMI/SED status, SSI/SSDI enrollment, and medicaid enrollment.
  7. Diagnosis Category by Source of Referral: Number of people served in each diagnosis category by source of referral.
